Contact Us If you have an inquiry, complete the questionnaire below and press Submit. This estate planning form is FOR TEXAS RESIDENTS ONLY! Our estate planning questionnaire is a simple form that allows you to receive a response by email or letter (your choice) which will not only tell you what documents you need in your estate plan but also will tell you exactly what it will cost to have Mr. Davis prepare such documents for you! All information will be held in confidence by the firm. MY PLEDGE TO YOU... After I receive your questionnaire, my firm will send you preliminary recommendations and the exact fee you will be charged if you choose to have my firm prepare your estate plan. Before we prepare any documents for you, however, (i) we will forward an engagement letter to you for your signature, (ii) we will require a personal or telephonic discussion with you to obtain such additional information as may be required in order to give you the correct legal advice, and (iii) we will require a retainer equal to half of the fees quoted to you. There will be no pressure and no intimidation. If you do not wish to proceed to have the recommended estate plan prepared for you, simply do not sign the engagement letter and you will not be billed for my recommendations or any discussions with me. I want to make the preparation of your estate plan as easy for you as possible. Appointments to meet and discuss your situation are also available. Answer all questions as accurately as possible. If you have any questions, please contact us. KEITH E. DAVIS, ATTORNEY AT LAW Fax: 214-291-5965 All information will be kept CONFIDENTIAL. Our Disclaimer: No attorney client relationship between you and our law firm will be established until we review your Questionnaire, send you a reply and an engagement letter, and a copy of the engagement letter and payment of the retainer quoted in that letter is returned to my firm. Any recommendations we send to you will be preliminary only and you agree that no documents will be signed by you until Mr. Davis has first had an opportunity to discuss the documents with you and has obtained such additional information as he may deem necessary in order to make a final recommendation regarding your will, trust or estate plan.
